  • CLTS is a Private Based Organization (PBO), therefore to proceed further in this process with CLTS all prospective members are required to complete an Affidavit of Knowing (AOK) Agreement. The completion of your AOK form is required prior to proceeding with any form of CLTS meeting, group event and/or CLTS resource access.

    To complete your Electronic AOK Agreement, please follow the instructions below in regards to the digital form.

Once you’ve completed Step 1 above by submitted your Electronic AOK Agreement, here is what to expect…

  • Once you formally submit your Electronic AOK Agreement via Step 1 referenced above, you will be automatically directed to an AOK Completion Webpage. This AOK Completion Webpage is your confirmation that CLTS has formally received your electronic AOK Agreement and no further action is required on your part.

    Upon receipt of your electronic AOK Agreement, your CLTS representative will follow up with you in regards to your next steps.
